A comprehensive examination of seven prestigious sleep medicine journals was conducted to identify randomized controlled trials (RCTs) published between 2010 and 2020. RCT abstracts with primary outcome findings that were statistically insignificant were included in the analysis of 'spin,' employing pre-defined strategies for 'spin' identification. To explore the association between the characteristics of the included abstracts and the presence and severity of 'spin', logistic regression analyses or chi-square tests were applied.
This study's dataset comprised 114 RCT abstracts; 89 of these (78.1%) exhibited the presence of at least one 'spin' strategy. Regarding the Results section, 66 abstracts (579%) displayed the word 'spin', while 82 abstracts (719%) presented 'spin' in their Conclusions. The 'spin' effect showed significant variation in different RCTs according to the specific research category (P=0.0047) and the presence/absence of statisticians (P=0.0045). More than half of haemodialysis patients succumb within a five-year period. Poor survival outcomes are associated with acute and chronic disturbances in salt and fluid homeostasis, and these are established as individual risk factors for mortality. Yet, the way their actions affect their mortality remains unexplained.
Our retrospective cohort analysis, guided by the European Clinical Database 5, assessed the correlation between transient hypo- and hypernatremia, fluid status, and mortality risk in a cohort of 72,163 haemodialysis patients hailing from 25 countries. Patients initiated on hemodialysis, each with a minimum of one valid bioimpedance spectroscopy measurement taken between January 1, 2010 and December 4, 2020, were observed until their death or removal due to administrative procedures. Fluid overload was diagnosed when the volume of fluids surpassed 25 liters above the norm, and fluid depletion was diagnosed when the volume fell short by 11 liters of the normal fluid status. Monthly plasma sodium and fluid status measurements, available for N=2272041, were subjected to Cox regression analysis for time-to-death.
Mortality risk from hyponatremia (plasma sodium below 135 mmol/L) was marginally higher when fluid status was normal (hazard ratio 126, 95% confidence interval 118-135), increased by half when patients were in a state of fluid depletion (hazard ratio 156, 95% confidence interval 127-193), and dramatically increased during fluid overload (hazard ratio 197, 95% confidence interval 182-212).
The risk of death is independently elevated by plasma sodium levels and fluid status. Fluid management observation of patients, notably those at high risk for hyponatremia, is of exceptional significance. Further studies on patient populations should explore the impacts of chronic hypo- and hypernatremia, risk-determining factors, and the resulting health risks.
