Clinical bacterial isolates' whole genome sequencing (WGS) holds transformative potential for both diagnostics and public health. To achieve this potential, a bioinformatic software solution is required that, in reporting identification results, aligns with the quality standards of a diagnostic test. K-mer-based strategies formed the basis for GAMBIT (Genomic Approximation Method for Bacterial Identification and Tracking), our new tool for bacterial identification from whole-genome sequencing (WGS) reads. A searchable database of 48224 genomes, meticulously curated, is incorporated into GAMBIT's algorithm. We present here the validation of the scoring methodology, the parameter's robustness, the determination of confidence levels, and the building of the reference database. We validated GAMBIT, a laboratory-developed test, in two public health labs during its deployment. The high incidence of bacterial infections is a substantial driver of illness and death among cirrhotic patients. Our study sought to quantify the rate of bacterial infections, notably those resulting from multidrug-resistant organisms (MDROs), both before and after the launch of the Stewardship Antimicrobial in VErona (SAVE) program. A cohort of 229 cirrhotic patients, without prior hospitalizations for infections, enrolled at the University of Verona Hospital between 2017 and 2019, were followed through December 2021. The mean duration of follow-up was 427 months.
