A digital quasi-experimental study, conducted in Japan, engaged 174 midwives and nurses possessing prior work experience in obstetrics or pediatrics. This prospective, multi-center, randomized interventional study compared ultrasound-guided versus fluoroscopy-guided lumbar medial branch blocks (LMBBs) in terms of their analgesic effectiveness and impact on disability in patients with pain originating from lumbar facet joints (LFJs).
Fifty adults with LFJ syndrome were randomly divided into two groups. The fluoroscopic group (FS) used fluoroscopic guidance to block the medial branch at the three lumbar levels L3-L4, L4-L5, and L5-S1. The ultrasound group (US) used ultrasound for the identical blocks. Both methods shared the use of a needle positioned transversely. The subjects' pain, disability, and activity levels were evaluated by the Visual Analogue Pain Scale (VAPS), Oswestry Disability Index (ODI), and Duke's Activity Status Index (DASI), at the start of the treatment, and one week and one month after treatment. The HADS (Hospital Anxiety and Depression Scale) score was gathered from the patient prior to the start of the procedure. Statistical analyses included variance analysis, one-sided and two-sided Mann-Whitney U tests, and the Chi-square test.

Via an enzyme-linked immunosorbent assay, the SF samples were evaluated for the presence of HMGB1, IL-1, IL-18, PGE2, RAGE, TLR4, and iNOS. To evaluate the therapeutic effect of HA, a comparison of pre-treatment and post-treatment clinical symptoms was performed in TMJOA patients who underwent intra-articular HA injections.
